NEW PASSPORTS - ADULTS BORN IN THE US

For U.S.-born adults of Haitian parent(s) seeking a Haitian passport

Required Documents

For the Applicant:

  • US birth certificate

  • Notarized French translation of the birth certificate

  • U.S. passport

  • Notarized letter (in English and French) explaining your request for a Haitian passport

  • Notary public signature must be authenticated by a court (e.g., Supreme Court of New York)

Witnesses Required:

  • Two Haitian witnesses with:

  • Valid Haitian passports

  • State-issued ID

  • Witnesses must be older than the applicant

For the Parents:

  • Haitian birth certificate or Extract from Archives

  • Haitian passport(s)

  • Parents' marriage certificate (if applicable)

  • Copy of parent's State-issued ID

Important

  • The applicant and both witnesses must appear in person at the Consulate

  • All documents must be submitted with originals and copies
